Message from Pete Moore

I have been living with long term health conditions since a child and learned to live with each of them over the years. I had to learn how to self manage and adapt to them, so I could live life to the best of my abilities.

Self management is a confusing term and for many people, when told to by a healthcare professional they think they have to have to work it out on their own. So, in recent years the word supported has been included, so that people don't thing they have to do it all themselves.

So what is What is supported self-management?

Supported self-management means increasing the knowledge, skills and confidence a person (with the support of a healthcare professional) has in managing their own health and care by putting in place interventions such as: peer support, self-management education and health coaching.

Supported self management coaching
